Responsibilities
- Provide compassionate in-person patient care including bathing, dressing, and mobility assistance
- Assist with telehealth coordination and virtual patient monitoring
- Document patient care accurately in our digital health records system
- Monitor vital signs and report changes to clinical team
- Coordinate care schedules and appointments via our remote portal
- Support patients with medication reminders and health education
- Maintain HIPAA compliance in all digital and physical interactions
Qualifications
- Active Pennsylvania CNA certification required
- Minimum 1 year clinical experience preferred
- Basic computer proficiency for telehealth platforms
- Strong communication and empathy skills
- Ability to work flexible shifts including evenings/weekends
- Current CPR/BLS certification
- Clear background check and drug screening
- Reliable transportation for in-person visits
