Responsibilities
- Direct Patient Support: Assist patients with daily living activities such as bathing, dressing, grooming, and toileting in a respectful and dignified manner.
- Vital Signs Monitoring: Accurately measure and record patient temperature, pulse, respiration rate, and blood pressure as per nursing standards.
- Assistance with Mobility: Safely transfer patients using Hoyer lifts or manual assistance, ensuring patient comfort and preventing injury.
- Hygiene & Sanitation: Maintain a clean, safe, and therapeutic environment by ensuring beds are made and linens are changed regularly.
- Communication: Effectively communicate patient needs and changes in condition to the Registered Nurse (RN) and interdisciplinary team.
- Charting: Document all care provided accurately and timely in the Electronic Health Record (EHR) system.
Qualifications
- Licensure: Valid and active Certified Nursing Assistant (CNA) license issued by the State of Minnesota.
- Education: High School Diploma or GED required; completion of an accredited CNA training program is mandatory.
- Certification: Basic Life Support (BLS) certification is preferred; we provide training if needed.
- Experience: Previous experience in a hospital, long-term care, or home health setting is highly preferred.
- Physical Ability: Ability to stand for long periods, lift up to 50 lbs, and perform physical assistance tasks as required by the job.
- Soft Skills: Strong interpersonal skills, patience, and a genuine passion for patient-centered care.
