Responsibilities
- Provide compassionate patient care including bathing, dressing, and feeding assistance
- Monitor vital signs and report changes to nursing staff
- Assist with mobility transfers and ambulation
- Document patient care accurately in electronic health records (remote component available)
- Collaborate with interdisciplinary care teams
- Maintain a clean and safe patient environment
- Support telehealth monitoring for remote patient check-ins
Qualifications
- Valid New York State CNA certification required
- Minimum 1 year clinical experience preferred
- BLS/CPR certification
- Strong communication and interpersonal skills
- Ability to work flexible shifts including weekends
- Basic computer proficiency for remote documentation
- Pass background check and drug screening
