Responsibilities
- Provide daily assistance with personal hygiene, mobility, and medication administration
- Monitor vital signs and report changes to nursing staff
- Assist with meal preparation and feeding support
- Document patient care accurately and maintain confidential records
- Support recreational activities and social engagement programs
- Collaborate with healthcare teams to develop individualized care plans
Qualifications
- Active Illinois CNA certification in good standing
- Minimum 1 year of long-term care experience preferred
- Current CPR/BLS certification
- Ability to lift up to 50 pounds and stand for extended periods
- Strong communication and interpersonal skills
- Background check and drug screening required
