Responsibilities
- Provide direct patient care, including bathing, dressing, and grooming.
- Monitor vital signs and report any changes in patient condition to the RN or supervisor.
- Assist with mobility and transfers using proper body mechanics.
- Maintain accurate and timely documentation of patient care plans and progress notes.
- Ensure a clean, safe, and infection-free environment for residents and clients.
- Communicate effectively with patients, families, and the interdisciplinary care team.
Qualifications
- Current and active Certified Nursing Assistant (CNA) or Licensed Practical Nurse (LPN) license in Arizona.
- Current Basic Life Support (BLS) and CPR certification.
- High school diploma or GED required.
- Previous experience in home health or long-term care is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
