Responsibilities
- Deliver high-quality virtual patient care and health monitoring via telehealth platforms
- Assist with daily living activities and medication reminders through digital coordination
- Maintain accurate electronic health records (EHR) documentation
- Collaborate with interdisciplinary teams to ensure seamless patient care
- Provide emotional support and health education to patients and families remotely

Qualifications
- Active Arizona CNA certification in good standing
- Minimum 1 year of clinical experience in home health or long-term care
- Proficient with telehealth software and electronic health records
- Strong communication skills with ability to build rapport virtually
- Ability to work independently while maintaining HIPAA compliance
- Valid CPR certification
