Responsibilities
- Provide direct patient care and assistance with Activities of Daily Living (ADLs) including bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, temperature, and pulse, reporting any abnormalities to the RN.
- Assist patients with mobility issues, transfers, and using mobility aids (walkers, wheelchairs).
- Administer medications as directed by the Charge Nurse and document all medication administration accurately.
- Maintain a clean, safe, and organized patient environment, adhering to infection control protocols.
- Communicate effectively with patients, families, and the interdisciplinary healthcare team to ensure holistic care.
- Respond promptly to patient needs and emergencies, demonstrating empathy and clinical expertise.
Qualifications
- Current and active Wisconsin CNA Certification (required).
- Current Basic Life Support (BLS) Certification for Healthcare Providers (AHA).
- Strong interpersonal and communication skills with a patient-centered approach.
- Ability to stand for extended periods and lift up to 50 lbs (assisting patients).
- High school diploma or GED required.
- Must pass a background check and drug screen.
- Previous experience in a long-term care or hospital setting is preferred but not mandatory.
