Responsibilities
- Provide exceptional personal care and assistance with Activities of Daily Living (ADLs) including bathing, dressing, and grooming.
- Monitor and accurately record patient vital signs, such as blood pressure, temperature, pulse, and respiration rates.
- Assist with mobility, transfers, and wheelchair assistance to ensure patient safety.
- Prepare and serve meals according to specific dietary restrictions and nutritional plans.
- Maintain a clean, organized, and safe patient environment in accordance with health and safety regulations.
- Communicate effectively with patients, families, and the nursing team regarding patient status and needs.
- Document care provided in electronic health records (EHR) and shift reports promptly.
Qualifications
- Current and valid State of Michigan CNA Certification (CNA 1) required.
- Current Basic Life Support (BLS) and CPR certification.
- Previous experience in home care, hospice, or a long-term care facility is preferred.
- Strong interpersonal and communication skills with a patient-centered approach.
- Ability to lift, push, and pull patients safely (up to 50 lbs) as needed.
- Reliable transportation and valid driver’s license.
