Responsibilities
- Provide high-quality direct patient care, including assistance with bathing, dressing, and grooming.
- Monitor and record patient vital signs, such as blood pressure, pulse, and temperature, accurately and timely.
- Assist patients with mobility, transfers, and ambulation using proper body mechanics and lifting techniques.
- Administer medication and treatments as directed by the Registered Nurse (RN) or Nurse Practitioner (NP).
- Maintain a clean, safe, and therapeutic environment for patients and staff.
- Communicate effectively with patients, families, and the interdisciplinary healthcare team regarding patient status.
- Document all care activities and patient observations in the electronic health record (EHR).
Qualifications
- Valid New York State (NYS) Certified Nursing Assistant (CNA) license and registration.
- Current Basic Life Support (BLS) Certification for Healthcare Providers.
- Previous experience in a hospital, nursing home, or home health setting is preferred but not required.
- Strong interpersonal and communication skills with a focus on patient-centered care.
- Ability to stand for extended periods and lift up to 50 lbs safely.
- Reliable transportation to various shift locations across New York.
