Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Monitor and accurately record vital signs, including blood pressure, temperature, pulse, and respiration rates.
- Provide emotional support and companionship to patients and their families during difficult times.
- Assist with mobility and safely transfer patients from beds to wheelchairs or walkers.
- Prepare and serve meals according to specific dietary restrictions and preferences.
- Observe and report any changes in a patient’s physical condition or behavior to the Registered Nurse or Case Manager immediately.
- Assist with light housekeeping tasks related to patient care, such as changing linens.
Qualifications
- High School Diploma or GED required.
- Must be willing to obtain State Tested Nursing Assistant (STNA) or CNA certification (we offer guidance and support).
- Strong interpersonal and communication skills with a compassionate demeanor.
- Ability to lift, push, and pull patients weighing up to 250 lbs safely.
- Valid driver’s license and reliable transportation are required for home visits.
- Basic computer skills and ability to use electronic medical record (EMR) systems.
