Responsibilities
- Provide direct, compassionate patient care and assistance with Activities of Daily Living (ADLs) such as bathing, grooming, and dressing.
- Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, reporting any abnormalities immediately.
- Assist patients with mobility, transfers, and positioning to ensure patient safety and comfort.
- Communicate changes in patient conditions or concerns to the Registered Nurse (RN) or Charge Nurse promptly.
- Maintain a clean, safe, and sanitized patient environment in accordance with health department standards.
- Document patient care activities, intake/output, and behavioral changes accurately and in a timely manner.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
Qualifications
- Current, unencumbered California State CNA license.
- Basic Life Support (BLS) certification (American Heart Association) required.
- Previous experience in a hospital, skilled nursing facility, or home health setting is preferred.
- Strong communication skills and the ability to work effectively in a fast-paced environment.
- Flexibility to work rotating shifts, including nights, weekends, and holidays.
- Physical stamina and the ability to lift, push, and pull patients weighing up to 250 lbs.
- Valid California Driver's License and reliable transportation.
