Responsibilities
- Provide virtual patient assessments via video calls during assigned night shifts
- Monitor vital signs and medication adherence through telehealth platforms
- Coordinate with on-site care teams for urgent interventions
- Document care plans accurately in HIPAA-compliant systems
- Offer emotional support and health education to patients/families
- Respond to emergency alerts within 15-minute SLA
- Participate in interdisciplinary case reviews
Qualifications
- Active Arizona CNA certification required
- 1+ years clinical experience (home health preferred)
- Reliable internet connection (min 25 Mbps)
- Experience with telehealth software (e.g., Teladoc, Amwell)
- Strong communication skills in English/Spanish a plus
- Ability to work independently during night hours
- Valid US driver's license (for occasional in-person visits)
- BLS/CPR certification
