Responsibilities
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Prepare nutritious meals and assist with feeding as needed.
- Provide medication reminders and documentation of care logs.
- Offer companionship and emotional support to clients to reduce loneliness.
- Perform light housekeeping and laundry tasks to maintain a safe living environment.
- Accompany clients to medical appointments and errands (Travel Companion duties).
- Monitor client health and report any changes or concerns to the Case Manager immediately.
Qualifications
- High school diploma or GED required.
- Current Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred but not mandatory (training provided).
- Valid Colorado driver's license and reliable transportation.
- Ability to pass a background check and drug screening.
- CPR and First Aid certification.
- Strong interpersonal skills and a genuine, patient demeanor.
- Physical ability to lift, bend, and stand for extended periods.
