Responsibilities
- Deliver personalized in-home care including bathing, meal preparation, and medication management
- Monitor client health status and document care activities via our secure digital platform
- Coordinate telehealth appointments and virtual family check-ins
- Transport clients to medical visits using company-provided vehicles
- Implement personalized care plans with remote oversight from clinical supervisors
- Maintain accurate electronic health records with remote accessibility
Qualifications
- High school diploma or equivalent required
- Valid Texas driver's license with clean driving record
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification
- Experience with telehealth platforms and electronic documentation systems
- Ability to pass background check and TB screening
- Strong communication skills for remote client interactions
