Responsibilities
- Provide direct personal care assistance to clients, including bathing, dressing, grooming, and toileting.
- Assist clients with mobility, transferring, and walking using proper body mechanics and assistive devices.
- Prepare nutritious meals and assist clients during meal times.
- Monitor clients' health status, reporting any changes in condition or symptoms to the Registered Nurse or Case Manager immediately.
- Accompany clients to medical appointments, therapy sessions, and social activities as needed.
- Light housekeeping duties related to client care, such as changing linens and laundry.
- Administer medication reminders and document care logs accurately.
Qualifications
- High School Diploma or GED is required.
- Current Home Health Aide (HHA) certification or Certified Nursing Assistant (CNA) license preferred.
- Must possess a valid Driver’s License and reliable transportation for client travel.
- Excellent communication skills and a patient, empathetic demeanor.
- Ability to pass a background check and drug screening.
- Current Basic Life Support (BLS) certification is a plus.
