Responsibilities
- Provide virtual patient care coordination and health monitoring
- Conduct telehealth assessments and document patient vitals
- Assist patients with medication management via digital platforms
- Coordinate care plans with RNs and physicians remotely
- Deliver compassionate patient education through video calls
- Maintain accurate electronic health records (EHR)
- Respond to patient inquiries via secure messaging systems
Qualifications
- High school diploma or equivalent required
- No prior experience necessary – we train!
- Basic computer proficiency with reliable internet
- Strong communication and empathy skills
- Ability to pass background check and drug screening
- Washington state CNA certification (or willingness to obtain)
