Responsibilities
- Provide telehealth assessments and patient consultations via secure digital platforms
- Monitor patient health data and coordinate care with multidisciplinary teams
- Administer medications and treatment plans remotely under physician supervision
- Document patient encounters accurately in electronic health records (EHR)
- Educate patients on health management and preventive care
- Collaborate with case managers to ensure continuity of care
- Respond to patient inquiries and triage urgent situations

Qualifications
- Active RN license in Texas (or compact state with quick endorsement)
- Associates or Bachelor's degree in Nursing (ADN/BSN)
