Responsibilities
- Provide remote patient assessments via video consultations and digital monitoring tools
- Coordinate care plans with on-site clinical teams and specialists
- Administer medications and treatments following telehealth protocols
- Document patient interactions accurately in Epic EHR system
- Respond to patient inquiries through secure messaging platforms
- Collaborate with case managers for chronic disease management
- Participate in virtual interdisciplinary care conferences
- Maintain HIPAA compliance in all digital communications
Qualifications
- Associate's or Bachelor's degree in Nursing (BSN preferred)
- Active California Registered Nurse license
- 0-2 years of clinical nursing experience
- BLS certification from American Heart Association
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- Strong critical thinking and digital literacy skills
- Ability to work independently in a virtual setting
- Excellent written and verbal communication skills
