Responsibilities
- Deliver high-quality patient care through telehealth and in-home visits
- Collaborate with multidisciplinary teams to develop patient care plans
- Accurately document patient records using our EHR system
- Provide health education to patients and families
- Monitor patient progress and adjust care as needed
- Participate in continuing education and professional development
- Adhere to HIPAA compliance and telehealth protocols
Qualifications
- Active RN license in Colorado (or compact state)
- BSN degree preferred (ADN candidates considered)
- 0-2 years of clinical experience (new graduates welcome)
- BLS certification (required)
- Strong communication and critical thinking skills
- Comfortable with remote technology and telehealth platforms
