Responsibilities
- Deliver virtual patient assessments and telehealth consultations
- Develop and implement personalized care plans for chronic disease management
- Coordinate transitions of care between providers and facilities
- Maintain accurate electronic health records using Epic systems
- Provide patient education via secure video platforms
- Collaborate with care teams on treatment protocols
- Participate in continuous quality improvement initiatives
Qualifications
- Valid AZ RN license (or ability to obtain within 30 days)
- BSN degree from accredited program (ADN candidates with experience considered)
- Basic Life Support (BLS) certification
- Experience with telehealth technology (training provided)
- Strong critical thinking and assessment skills
- Excellent written and verbal communication abilities
- Ability to work independently in a remote environment
