Responsibilities
- Conduct Comprehensive Assessments: Evaluate patient symptoms via phone or secure video to determine the urgency of care and appropriate medical interventions.
- Medical Decision Making: Utilize clinical protocols to prioritize patient needs and facilitate timely referrals to physicians or urgent care facilities.
- Health Education: Provide clear, empathetic guidance on self-care, medication management, and follow-up instructions to patients and their families.
- Documentation: Accurately record patient interactions, assessments, and care plans in the electronic health record (EHR) system in real-time.
- Collaboration: Act as a liaison between patients, primary care providers, and specialists to ensure continuity of care.
Qualifications
- Licensure: Current, unencumbered Registered Nurse (RN) license in the District of Columbia or compact state eligibility.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required; CEN (Certified Emergency Nurse) preferred.
- Experience: Minimum of 2-3 years of acute care or triage nursing experience required.
- Communication: Exceptional verbal and written communication skills with the ability to de-escalate tense situations calmly.
- Technical Proficiency: Comfortable navigating EMR/EHR systems and telehealth platforms.
