Responsibilities
- Conduct comprehensive telehealth and in-person patient assessments to determine urgency of care needs
- Collaborate with physicians to develop appropriate treatment plans and triage protocols
- Provide clear medical guidance to patients via phone, email, and secure messaging
- Maintain accurate electronic health records (EHR) documentation
- Coordinate care transitions between emergency departments and specialty clinics
- Participate in continuous quality improvement initiatives
Qualifications
- Active NJ RN license with 3+ years clinical experience
- BSN or higher degree preferred
- BLS/ACLS certification required
- Triage certification or equivalent experience
- Strong assessment and decision-making skills
- Experience with Epic or EHR systems
- Ability to work independently and in fast-paced environments
