Responsibilities
- Conduct comprehensive telephonic and in-person patient assessments to determine urgency of care needs
- Utilize Epic EHR systems to document patient interactions and triage decisions with 100% accuracy
- Collaborate with physicians to develop appropriate care plans for acute and chronic conditions
- Provide compassionate patient education on treatment protocols and home care instructions
- Maintain strict HIPAA compliance and data security protocols for all patient communications
- Participate in continuous quality improvement initiatives for triage protocols
- Act as clinical resource for patients and families navigating complex healthcare systems
Qualifications
- Active RN license in Massachusetts with minimum 3 years clinical experience
- BLS/CPR certification and ACLS preferred
- Previous triage or emergency department experience required
- Proficiency in Epic EHR and telehealth platforms
- Exceptional critical thinking and decision-making abilities
- Strong communication skills with demonstrated empathy
- Ability to work independently with minimal supervision
