Responsibilities
- Provide high-quality patient assessment and care coordination via telehealth platforms
- Collaborate with interdisciplinary teams to develop and implement care plans
- Document patient encounters accurately in EMR systems with strict HIPAA compliance
- Manage caseloads across diverse specialties including critical care, telemetry, and med-surg
- Maintain clinical excellence through continuous education and quality improvement initiatives
- Respond to patient inquiries and triage urgent situations remotely
- Participate in peer review and quality assurance programs
Qualifications
- Active CA RN license with no disciplinary actions
- 2+ years of acute care experience in hospital setting
- Proficiency in telehealth technology and EMR systems (Epic, Cerner)
- BLS/ACLS certification required; PALS preferred
- Exceptional communication skills with remote work aptitude
- Ability to adapt to flexible schedules including nights/weekends
- Strong clinical judgment and problem-solving abilities
- Experience with chronic disease management preferred
