Responsibilities
- Provide comprehensive telehealth assessments and patient consultations via HIPAA-compliant platforms
- Collaborate with care teams to develop and implement personalized treatment plans
- Monitor patient progress through electronic health records and adjust interventions as needed
- Coordinate care transitions between facilities and community resources
- Educate patients and families on chronic disease management and preventive care

Qualifications
- Active RN license in Massachusetts (compact license acceptable)
- Minimum 2 years clinical nursing experience
