Responsibilities
- Assess patient calls via phone, email, and telehealth platforms to determine urgency and appropriate care pathways
- Document clinical assessments accurately in Epic EHR system following HIPAA protocols
- Collaborate with physicians and care teams to coordinate patient transfers and admissions
- Provide health education and discharge instructions to patients and families
- Participate in continuous quality improvement initiatives for triage protocols
- Maintain crisis intervention skills for emergency response situations
- Mentor new triage staff during orientation periods
Qualifications
- Active RN license in New York State with minimum 2 years clinical experience
- ACLS/BLS certification with current credentials
- Proficiency in telephone triage protocols (e.g., Triage Logic or similar systems)
- Strong assessment skills for diverse patient populations across age groups
- Exceptional communication skills with ability to de-escalate anxious callers
- Experience with electronic health records (Epic preferred)
- Flexibility to work rotating shifts including nights, weekends, and holidays
- Telehealth experience highly desirable
