Responsibilities
- Patient Triage: Perform detailed symptom assessments via phone and video to determine the urgency of medical needs and recommend appropriate care pathways.
- Clinical Decision Making: Apply critical thinking skills to evaluate patient history and current conditions to determine the necessity of ER visits or home care.
- Documentation: Maintain accurate and up-to-date patient records in the EMR system, ensuring all interactions are logged per HIPAA standards.
- Resource Coordination: Facilitate referrals to specialists, urgent care centers, or primary care physicians when necessary.
- Telehealth Support: Provide empathetic and clear communication to patients through virtual platforms, ensuring they feel heard and understood.
- Emergency Protocols: Adhere to strict triage protocols to ensure high-risk patients are immediately escalated to emergency services.
- Quality Assurance: Participate in case reviews and quality improvement initiatives to enhance triage accuracy.
Qualifications
- Licensure: Current and valid RN license issued by the State of California (CA).
- Experience: Minimum of 3 years of nursing experience, with at least 1 year specifically in triage or telehealth nursing.
- Certifications: Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are mandatory.
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Communication: Superior verbal and written communication skills with a strong focus on patient empathy and conflict resolution.
- Technical Proficiency: Must be comfortable navigating Electronic Medical Records (EMR) and telehealth software.
- Schedule Availability: Must be willing to work flexible shifts, including evenings, nights, and weekends.
