Responsibilities
- Assess patient calls using standardized triage protocols to determine medical urgency
- Provide accurate clinical advice and health education to patients via phone/video
- Collaborate with physicians and care teams for complex case management
- Maintain detailed electronic health records (EHR) documentation
- Coordinate referrals and follow-up care across healthcare continuum
- Adhere to HIPAA compliance and privacy regulations
- Participate in continuous quality improvement initiatives
Qualifications
- Active Virginia RN license with unrestricted status
- Minimum 2 years clinical experience in acute care/urgent setting
- BLS certification; ACLS preferred
- Proficiency in EHR systems (Epic/EMR experience)
- Exceptional communication and critical thinking skills
- Ability to work flexible shifts including nights/weekends
- Triage certification or equivalent training
- Strong problem-solving under pressure
