Responsibilities
- Deliver personalized patient care including bathing, dressing, and mobility assistance
- Monitor vital signs and document patient observations accurately
- Assist with medication administration under RN supervision
- Collaborate with interdisciplinary care teams to optimize patient outcomes
- Maintain a clean, safe, and infection-free patient environment
- Provide emotional support to patients and families during healthcare transitions
- Participate in patient care planning and team meetings
Qualifications
- Active Colorado CNA certification required
- Minimum 1 year recent acute care or long-term experience
- BLS/CPR certification (American Heart Association)
- Ability to lift up to 50 pounds and stand for extended periods
- Strong communication and interpersonal skills
- Proficiency in electronic health record systems
- Pass background check and pre-employment physical
- Valid Colorado driver's license (if transporting patients)
