Responsibilities
- Provide remote patient monitoring and virtual health assessments via telehealth platform
- Coordinate care plans with RNs and physicians using our HIPAA-compliant digital tools
- Conduct virtual medication reconciliation and health education sessions
- Document patient interactions accurately in our EMR system
- Collaborate with on-site staff during patient transitions and emergencies
- Participate in interdisciplinary team meetings remotely
- Maintain strict confidentiality and compliance with healthcare regulations
Qualifications
- Active California CNA certification required
- Minimum 1 year acute care or long-term experience
- Proficiency with telehealth platforms (Zoom, Doxy.me, etc.)
- Strong documentation skills in EMR systems
- Excellent communication and empathy skills
- Ability to work independently with minimal supervision
- Flexibility to cover varied shifts including evenings/weekends
- Basic computer literacy and reliable home internet connection
