Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting.
- Monitor and accurately report any changes in a patient's physical or mental condition to the supervising nurse or case manager.
- Prepare nutritious meals and assist with light housekeeping to maintain a safe, clean, and comfortable living environment.
- Provide emotional support and companionship to patients to reduce feelings of isolation and improve quality of life.
- Accompany patients to medical appointments, therapy sessions, or social activities as required.
- Assist with medication reminders and ensure all personal hygiene needs are met.
- Transport patients to and from appointments using personal vehicle.
Qualifications
- High School Diploma or GED is required.
- Valid State Driver's License and reliable transportation are mandatory.
- CPR and Basic First Aid certification is highly preferred.
- Ability to pass a comprehensive background check and drug screening.
- Strong interpersonal and communication skills with a compassionate demeanor.
- Physical ability to lift, stand, walk, and bend for extended periods.
