Responsibilities
- Provide comprehensive telehealth assessments and patient consultations via HIPAA-compliant platforms
- Develop and implement individualized care plans for remote patient populations
- Coordinate multidisciplinary virtual care teams for complex cases
- Monitor patient progress through remote monitoring systems and digital health records
- Educate patients on chronic disease management and medication adherence

Qualifications
- Active RN license in Maryland (or compact state with quick licensure)
- Bachelor of Science in Nursing (BSN) required, MSN preferred

- Certification in Telehealth Nursing (C-T) or willingness to obtain
- Expertise in chronic disease management and patient education
- Proficiency with EHR systems and telehealth platforms
- Exceptional communication skills for virtual patient interactions
- Ability to work independently with minimal on-site supervision
