Responsibilities
- Perform comprehensive telephone and in-person patient assessments to determine urgency and appropriate level of care.
- Utilize clinical decision-making skills to triage patients into appropriate care pathways (ER, Urgent Care, or home care).
- Document patient interactions and clinical findings accurately and timely in the Electronic Health Record (EHR) system.
- Counsel patients and families on health maintenance, disease prevention, and medication management.
- Coordinate referrals to specialists and follow up on patient outcomes to ensure continuity of care.
- Collaborate with physicians and healthcare providers to resolve complex clinical issues.
- Adhere to all HIPAA regulations and hospital protocols regarding patient privacy and safety.
Qualifications
- Current and valid Registered Nurse (RN) license issued by the State of California.
- Minimum of 2-3 years of nursing experience, preferably in a triage or emergency setting.
- ACLS and BLS certifications are mandatory.
- Strong communication skills with the ability to de-escalate tense situations over the phone.
- Proficiency in electronic medical record systems (Epic, Cerner, or similar).
- Ability to work flexible shifts, including evenings, weekends, and holidays.
- BSN or Associate Degree in Nursing preferred.
