Responsibilities
- Conduct comprehensive telephone and digital triage assessments for patients presenting a wide range of symptoms.
- Utilize evidence-based protocols to determine the appropriate level of care (Home care, Urgent Care, or ER).
- Document patient interactions accurately and securely within the EMR system.
- Collaborate with physicians and care managers to develop individualized treatment plans.
- Provide health education and referrals to appropriate community resources.
- Monitor call queues and manage high-volume communication channels efficiently.
Qualifications
- Current, unencumbered RN License to practice in Arizona (or compact state).
- Minimum of 2-3 years of nursing experience, preferably in a triage, telehealth, or acute care setting.
- Certification in Triage Nursing (CEN) or Medical-Surgical Nursing (MSN) preferred.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Exceptional communication skills with the ability to de-escalate sensitive situations.
- Proficiency in computer systems and EMR software (Epic, Cerner, etc.).
