Responsibilities
- Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and record vital signs such as blood pressure, pulse, and temperature.
- Provide companionship and emotional support to reduce feelings of isolation.
- Assist with light housekeeping and meal preparation according to dietary restrictions.
- Administer medication as directed by the Registered Nurse or case manager.
- Transport clients to medical appointments or errands using personal vehicle.
- Document patient care activities and changes in condition accurately in electronic health records.
Qualifications
- High School Diploma or GED required; CNA or HHA certification preferred.
- Current CPR and First Aid certification.
- Valid Massachusetts driver's license and clean driving record.
- Strong communication skills and ability to follow verbal and written instructions.
- Ability to lift at least 50 lbs and perform physical tasks required for patient care.
- Pass a criminal background check and drug screen.
- Previous experience in home health or geriatric care is a plus.
