Responsibilities
- Assist clients with activities of daily living (ADLs) including bathing, dressing, and grooming.
- Monitor and report changes in patient health, mood, or behavior to the RN or Case Manager.
- Prepare nutritious meals and assist with feeding as needed.
- Provide companionship and emotional support to patients.
- Perform light housekeeping and laundry related to patient care.
- Assist with mobility, transferring, and walking.
- Administer medication reminders as directed by the supervising nurse.
Qualifications
- High School Diploma or GED equivalent.
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification is strongly preferred.
- Current Basic Life Support (BLS) and CPR certification.
- Valid Driver’s License and reliable transportation.
- Strong communication and interpersonal skills.
- Ability to pass a background check and drug screen.
- Caring, patient, and reliable personality.
