Responsibilities
- Provide direct personal care assistance with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and record patient vital signs, such as weight, temperature, and blood pressure.
- Assist with mobility, transfers, and light exercise routines as prescribed by the nurse or physician.
- Prepare nutritious meals and assist with feeding patients who require special dietary modifications.
- Administer medication and remind patients to take their prescribed medicines on time.
- Document daily care logs and communicate any changes in patient condition to the supervising nurse immediately.
Qualifications
- High School Diploma or GED required.
- Current State Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) is preferred.
- Valid Driver’s License and reliable transportation required.
- CPR and First Aid certification (or willingness to obtain).
- Strong interpersonal skills and the ability to communicate effectively with patients and families.
- Ability to pass a background check and drug screen.
