Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Provide assistance with mobility and transfer using provided equipment (walkers, wheelchairs, hoyer lifts).
- Monitor and report changes in client behavior, health, or mood to the Case Manager.
- Prepare nutritious meals and assist with light housekeeping and laundry as assigned.
- Administer medication reminders and ensure clients take prescriptions as directed.
- Accompany clients to medical appointments and assist with errands.
- Document daily care logs and maintain accurate records.
Qualifications
- Valid California Home Health Aide (HHA) Certification or Certified Nursing Assistant (CNA) license.
- Current Basic Life Support (BLS) certification.
- Strong interpersonal and communication skills.
- Ability to pass a background check and drug screening.
- Physical stamina to lift, bend, and stand for extended periods.
- Reliable transportation and a valid driver's license.
- Compassionate nature with a genuine desire to help others.
