Responsibilities
- Personal Care Assistance: Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Medication Management: Remind clients to take prescribed medications at scheduled times.
- Companionship: Provide emotional support and engage clients in conversation and social activities to prevent isolation.
- Light Housekeeping: Maintain a clean, safe, and organized living environment for clients.
- Mobility Support: Help clients transfer in and out of bed or wheelchairs safely.
- Vital Signs Monitoring: Track and report changes in client health status to the nursing supervisor.
- Documentation: Accurately complete daily care logs and progress notes in our electronic health record system.
Qualifications
- Education: High school diploma or GED required.
- Certification: Current MA Home Health Aide certification and valid CNA license preferred.
- Experience: Previous experience in home health or long-term care is a plus but not mandatory.
- Skills: Strong interpersonal and communication skills with a patient-centered approach.
- Physical Ability: Ability to lift, bend, and stand for extended periods.
- Background Check: Must pass a comprehensive background check and drug screening.
