Responsibilities
- Provide personal care assistance to patients, including bathing, dressing, grooming, and oral hygiene.
- Assist patients with mobility issues, including transferring in and out of bed or wheelchairs, and walking with a walker or cane.
- Prepare nutritious meals and assist patients with feeding if necessary, adhering to dietary restrictions.
- Monitor and report any changes in a patient's physical condition, behavior, or mood to the supervising nurse or case manager.
- Perform light housekeeping tasks such as laundry, making beds, and vacuuming to ensure a safe and clean living environment.
- Accompany patients to medical appointments, therapy sessions, or for grocery shopping and errands.
- Administer medication reminders and document care activities accurately in patient records.
Qualifications
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) certification required.
- Current, unencumbered state licensure or valid certification.
- Valid Driver's License and reliable personal transportation.
- Strong interpersonal communication skills and a compassionate, patient-centered approach.
- Ability to pass a background check, drug screen, and physical examination.
- Physical stamina to stand, walk, lift, and assist patients for extended periods.
- Basic computer literacy for documentation purposes.
