Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting.
- Provide compassionate companionship and emotional support to improve the patient's quality of life.
- Monitor and record vital signs, such as blood pressure and pulse, and report any abnormalities to the nursing team.
- Prepare nutritious meals and assist with light housekeeping to maintain a clean and safe living environment.
- Transport clients to medical appointments, social events, and pharmacy runs as required.
- Administer medication reminders and assist with light medical tasks under the direct supervision of a licensed nurse.
- Document daily care logs and communicate effectively with families and case managers.
Qualifications
- Valid New York State Home Health Aide (HHA) or Certified Nursing Assistant (CNA) license.
- CPR and Basic Life Support (BLS) certification.
- High School Diploma or GED required.
- Strong interpersonal and communication skills with a patient-centered approach.
- Reliable transportation and a valid driver's license (preferred but not mandatory).
- Ability to pass a background check and drug screening.
- Physical stamina to assist patients with mobility and daily tasks.
