Responsibilities
- Personal Care: Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Medication Management: Remind clients to take prescribed medications and report any missed doses or adverse reactions to the nursing team.
- Household Support: Assist with light housekeeping, meal preparation, and laundry as assigned to ensure a safe and comfortable environment.
- Companionship: Engage clients in social activities, conversation, and cognitive exercises to enhance their emotional well-being.
- Transportation: Accompany clients to medical appointments, therapy sessions, and community events.
- Vital Signs: Monitor and record vital signs such as blood pressure, temperature, and pulse as directed.
Qualifications
- Certification: Current Home Health Aide (HHA) or Certified Nursing Assistant (CNA) license in the state of Wisconsin.
- Experience: Previous experience in home health, hospice, or long-term care preferred.
- Communication: Excellent verbal and written communication skills with the ability to interact compassionately with patients and families.
- Physical Requirements: Ability to lift, transfer, and assist patients with mobility; ability to stand or walk for extended periods.
- Valid Driver’s License: Reliable transportation is required for travel to client homes.
- Clean Background: Ability to pass a criminal background check and drug screen.
