Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting.
- Prepare nutritious meals and snacks based on prescribed dietary plans.
- Provide medication reminders and accurately document daily care logs.
- Accompany clients to medical appointments, therapy sessions, and social outings.
- Perform light housekeeping tasks such as laundry, vacuuming, and dishwashing.
- Monitor client health and report any changes or concerns to the RN Case Manager immediately.
Qualifications
- High School Diploma or GED required.
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
- Current CPR and Basic Life Support (BLS) certification.
- Valid driver’s license and reliable transportation required for client travel.
- Strong interpersonal communication skills and a patient, caring demeanor.
- Ability to pass a background check and drug screening.
