Responsibilities
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Prepare meals and assist patients with feeding as needed.
- Monitor and record vital signs, including blood pressure, pulse, and temperature.
- Perform light housekeeping duties to maintain a safe and clean living environment.
- Provide companionship and emotional support to patients and their families.
- Administer medication as directed by the Registered Nurse (RN) or Case Manager.
- Document patient care activities and report any changes in condition immediately.
Qualifications
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) certification required.
- Current Illinois state license (if applicable).
- Valid Driver's License and reliable transportation required.
- CPR and Basic Life Support (BLS) certification.
- Strong interpersonal and communication skills.
- Ability to pass a criminal background check and drug screening.
- Compassionate, patient, and dedicated attitude towards patient care.
