Responsibilities
- Conduct comprehensive virtual patient assessments via telehealth platform
- Develop and implement personalized care plans for chronic disease management
- Provide medication education and remote monitoring support
- Collaborate with interdisciplinary teams for coordinated care delivery
- Document patient encounters accurately in EHR systems
- Respond to urgent patient inquiries within SLA requirements
Qualifications
- Active CA RN license with unrestricted status
- 2+ years clinical experience in acute care or telehealth
- BLS/BLS Instructor certification required
- Proficiency with telehealth technology (Zoom for Healthcare, Epic)
- Strong critical thinking and crisis management skills
- Exceptional communication abilities across diverse populations
