Responsibilities
- Conduct thorough phone and in-person triage assessments to evaluate patient symptoms and determine medical acuity.
- Make evidence-based medical recommendations and facilitate timely appointments with appropriate providers.
- Document all patient interactions, assessments, and care plans accurately and securely in the EHR system.
- Collaborate closely with physicians, nurses, and other care team members to ensure seamless continuity of care.
- Provide clear patient education regarding symptoms, medication adherence, and wellness resources.
- Manage high-volume inquiries with professionalism, empathy, and efficiency.
Qualifications
- Current and active Registered Nurse (RN) license in the state of New York.
- Minimum of 2 years of nursing experience, with a preference for experience in triage or clinical settings.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong critical thinking, problem-solving, and decision-making skills under pressure.
- Excellent verbal and written communication skills.
- Proficiency with electronic health records (EHR) software.
