Responsibilities
- Provide exceptional first-line clinical support by accurately assessing patient symptoms via phone and secure messaging.
- Route patients to the appropriate level of care, including urgent care, emergency services, or primary physician referrals.
- Document all triage encounters accurately and promptly within the electronic health record system.
- Offer health education and resources to patients to promote self-care and wellness.
- Collaborate with the wider care team to ensure continuity of service for our community.
- Handle high-volume call volumes with patience, empathy, and professional composure.
Qualifications
- Current RN or LPN license in the State of Texas (or eligible for reciprocity).
- Graduation from an accredited nursing program.
- BLS (Basic Life Support) Certification required.
- Strong verbal communication skills with the ability to de-escalate difficult situations.
- Proficiency in Microsoft Office Suite and electronic health record systems.
- Ability to work flexible shifts, including evenings, weekends, and holidays.
