Responsibilities
- Conduct Patient Triage: Utilize standardized protocols to assess incoming patient calls and determine the appropriate level of care needed.
- Clinical Decision Making: Evaluate symptoms and symptoms to recommend home care instructions, medication, or immediate emergency services.
- Documentation: Accurately and timely update patient electronic health records (EHR) with assessment findings and care plans.
- Resource Coordination: Facilitate referrals to physicians, specialists, and other appropriate healthcare resources.
- Customer Service: Provide empathetic and clear communication to patients and families, ensuring they feel heard and supported.
- Policy Adherence: Comply with all hospital policies, HIPAA regulations, and quality assurance standards.
Qualifications
- Education: High School Diploma or GED required; Nursing Degree (ADN, BSN) or current enrollment in a nursing program is highly preferred.
- Licensure: Current RN or LPN license (or eligibility to obtain) in the State of California is required.
- Communication: Excellent verbal and written communication skills with the ability to explain complex medical information clearly.
- Technical Proficiency: Basic computer skills and comfort learning new medical software systems.
- Empathy & Professionalism: Strong ability to remain calm under pressure and demonstrate high emotional intelligence.
- Availability: Flexibility to work varying shifts, including weekends and holidays, as required by the department.
