Responsibilities
- Conduct thorough symptom assessments via phone to determine the appropriate level of care for patients.
- Evaluate patient history and current medical conditions to recommend appropriate interventions or home care instructions.
- Document all patient interactions accurately and promptly in the Electronic Health Record (EHR) system.
- Coordinate referrals to specialists or emergency services when necessary.
- Provide clear, empathetic instructions regarding medications and follow-up care.
- Assist in the development of triage protocols and patient education materials.
Qualifications
- Current, unencumbered RN or LPN license in the state of North Carolina.
- Graduate of an accredited Nursing program (New Graduates strongly encouraged to apply).
- Strong communication skills with the ability to de-escalate tense situations effectively.
- Basic computer proficiency and experience with EMR systems.
- Ability to work independently while maintaining close communication with the clinical team.
- Reliable transportation for occasional on-site meetings.
