Responsibilities
- Provide direct patient care, including assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, reporting any abnormalities to the RN or Charge Nurse.
- Assist with range-of-motion exercises and mobility support for patients with limited mobility.
- Maintain a clean, safe, and therapeutic environment for patients, ensuring bed linens and equipment are properly sanitized.
- Communicate effectively with patients, families, and the interdisciplinary healthcare team regarding patient status and care plans.
- Document patient care activities accurately and timely in electronic medical records (EMR).
Qualifications
- Certified Nursing Assistant (CNA) license in the State of Illinois required.
- Current Basic Life Support (BLS) certification.
- Previous experience in a hospital or long-term care setting is preferred but not mandatory.
- Strong interpersonal and communication skills with a patient-centered approach.
- Ability to stand for extended periods and lift up to 50 lbs.
- Must pass a background check and drug screen.
